  Use the line above to sign up online for membership in ILMSA/USMS.
• Retrieve your USMS Card
  If you are already an ILMSA or USMS member, you can use the link above to retrieve an electronic copy of your USMS card for the current year.
• Club/Team Registration Form [PDF, 135k]
  Use this form to register your team or club with ILMSA for 2008. Please note: Your club or team must be registered before its swimmers may register with that club or team for 2008.
• Individual Registration Form [PDF, 158k]
  Use this form if you are registering for the year, and your club does not do this for you (check with your coaching staff to see if your club handles your ILMSA registration).
• ILMSA Discount/Scholarship Form [PDF, 12k]
  If you are under the age of 26, and are a full-time student, you are eligible to receive a $25 discount on your ILMSA/USMS dues (making your total net dues a whopping $10). Just fill out the scholarship form (p. 1 of the document) and send it in with your registration. If you are a first-time ILMSA registrant under the age of 27 and not a full-time student, you are eligible to receive a $10 discount on your ILMSA/USMS dues (making your total net dues a whopping $25). Just fill out the form (p.2 of the document) and send it in with your registration.

  Use this form if you are transferring between clubs or teams during the year.
